Overview:

GENERAL SUMMARY OF POSITION: 

Under the direction of the Director of Practice Transformation, or designee, the Associate Director   manages operations and staff towards improving the clinical and operational performance of clinical practices and provider organizations implementing transformational clinical delivery models and preparing for value-based health care systems.  This will include such transformational models as Patient-Centered Medical Home (PCMH), clinical care management, behavioral health integration, and clinical system integration.  The Associate Director of Practice Transformation assists the Director in planning, reporting, evaluating, and facilitating the achievement of practice transformation and performance improvement.  He/she acts as a liaison between participating practice sites and all stakeholders associated with CWM’s Practice Transformation activities. The Associate Director manages a team of Practice Transformation Specialists/Quality Improvement Advisors, overseeing day-to-day practice transformation facilitation activities, as well as designing and implementing learning collaboratives and other forms of technical assistance for practices participating in healthcare delivery transformation initiatives.  Additionally, the Associate Director of Practice Transformation will provide input to practice transformation project leaders on the development, communication and implementation of a clear vision and strategy as to how practices will successfully transform and succeed in a value-based payment system.

Responsibilities:

MAJOR RESPONSIBILITIES:

  • Under the direction of the Director of Practice Transformation, provides leadership in planning, reporting, evaluating, and facilitating the achievement of practice transformation and performance improvement.
  • Provides input to practice transformation project leaders on the development, communication and implementation of:
    • a clear vision and strategy as to how practices will successfully transform and prepare for value-based payment models
    • an expected timeline for practice participants to implement the agreed upon strategy;
  • Oversees day-to-day technical assistance, coaching and facilitation activities of the team in a focused, accurate, and timely manner to ensure that participating practices are successful in their transformation efforts.
  • Responsible for the on-going training and development of staff to develop their leadership, quality improvement, consultative, oral presentation and change management skill sets.
  • Provides consultative supervision to Transformation Specialists/Quality Improvement Advisors from outside of UMMS per contractual or other arrangements.
  • Coordinates efforts of Transformation Specialists/Quality Improvement Advisors with the efforts of other practice transformation initiative stakeholders from within and outside UMMS.
  • Designs and oversees implementation of shared learning opportunities, such as learning collaboratives, for practices participating in transformational activities and other health care delivery reform initiatives.
    • Learning collaborative activities may include webinars, videos, panel discussions, in-person learning sessions, conference calls or office hours, posting of practice resources on a learning collaborative website.
  • Oversees all staff and external vendors to develop and refine transformation curricula and presentations using the learning modalities described above.
  • Ensures the quality of change package curriculum, presentations and shared learning activities throughout practice transformation initiatives.
  • Serves as a presenter in shared learning activities.
  • Working with the project team, coordinates the collection and analysis of data evaluating customer satisfaction and impact of the services of the Transformation Specialists/Quality Improvement Advisors and of the learning collaborative and utilizes this information to improve these transformation services.
  • Working with the project team, coordinates the collection, integration and evaluation of diverse data from participating practices related to clinical and operational performance and transformational activities; identifies trends and issues; and uses this information to inform transformational curricula and the work of the Transformation Specialists/Quality Improvement Advisors.
  • Communicates practice performance improvement and transformation activities, results and issues by developing reports with verbal, written and visual presentations to leadership teams, committees, and other key stakeholders.
  • Works with internal UMMS/Commonwealth Medicine administration to develop necessary business plans and financial analyses for practice transformation initiatives.
  • Works with internal UMMS/Commonwealth Medicine administration on business development opportunities related to practice transformation services, including responses for Request for Proposals.
  • Responsible for developing and overseeing budgets for practice transformation technical assistance services
  • Complies with established UMMS internal departmental policies, procedures, and objectives.
  • Demonstrates use of quality improvement best practices in daily operations.
  • Complies with all health and safety regulations and requirements.
  • Performs similar or related duties as required or directed.

Qualifications:

REQUIRED QUALIFICATIONS:

  • Master’s Degree in Nursing, Public Health, Business Administration, Education, or a related field.
  • A minimum of 6-8 years of related professional experience in quality/process improvement, and organizational change management, including experience in a clinical setting.
    • This includes a minimum of 3-5 years of progressively responsible management experience.
  • Experience in emerging models of care and practice transformation efforts, including PCMH, clinical care management and behavioral health integration.
  • Proven administrative and management ability in complex environments.
  • Experience with practice facilitation, including shared learning forums and practice-based technical assistance and coaching.
  • Demonstrated performance in program development and implementation.
  • Demonstrated ability to analyze and utilize data
  • Excellent oral and written communication skills.
  • Innovation, flexibility and creativity.
